This is the rarest of the 3 types of Apollo 11 Insurance Covers, Apollo 11 was the first mission to issue insurance covers as a way to provide money to the astronauts families should they not return from the moon. These covers were signed by all 3 of the crew of Apollo 11 some time before their flight in July 1969

Apollo 13 Flown EVA Prep Card
This Card is printed on very heavy cardstock,and measures 10.25 x 7.75 inches. And was intended to be used when Jim Lovell and Fred Haise were preparing to leave the Lunar Module sitting on the Lunar Surface at Fra Mauro, in order to perform their first EVA on the moon. However, the explosion of an oxygen tank in the command module, put an end to the lunar landing and alas this card serves as a reminder of what could have been.
